I read the new Lan rules or the revision of them.Do I need to fill a form if I play at other people's houses like for example my girlfriend's house but she doesn't play uo or anything?
so no multiple accounts(just my 1 account) or people playing just me logging in from my house and once in a while of my laptop at her house??

No,you don't unless you just log in with your account and your gf won't ever make an account on UOD

That thread is not about new rules. It's sounds more like a warning about refreshing accounts or logging them occasionaly
